Dental Insurance Company Denying Authorization for Covered Plan Services.?

Anthem Blue Cross Dental Net I was referred to an oral surgeon by my primary care dentist since I have to have 6 teeth and 2 residual root tips extracted in advance of getting a full upper denture. All the extractions including the root tips have to be done during one visit, which necessitates the use of IV sedation. My general dentist cannot perform any of this work. Anthem Blue Cross Dental Net denied the original pre-authorization request, and subsequent appeal stating that my general dentist can perform the services being referred. Which is absolutely not the case. I even spoke with a customer service rep who indicated that the extraction of residual root tips is definitely a referable service and it clearly states that in their Dental Net Provider Contract. She has no explanation for why the appeal was denied. My primary care dentist has even provided Anthem Blue Cross Dental Net with his specific reasons for referring this work to an oral surgeon and I still can't get anyone at Anthem Blue Cross to re-evaluate the original denial and the appeal and authorize the necessary work. I have filed a complaint with the California Department of Insurance, although I don't hold out much hope for any results from that effort. HELP... what are my options? Who can I contact? Thanks

My car insurance company (One Call) recently cancelled my policy due to them not receiving my proof of no claims in time (that's another issue). However, they failed to notify me in time and I carried on driving my car for 5 days un insured, until the letter arrived (policy cancelled on 10/9/12 and letter sent 14/9/12). Getting out of a speeding ticket for insurance purposes?

Is there a way I can get out of a speeding/failure to meet at a stop sign ticket? I really don't care about paying the ticket or court costs at all, but I just can not, really, really, can not pay the extra insurance money. I am paying out of state tuition AND I am in Architecture. Architecture is SUCH an expensive major. The police officer (whose name I can't even read on the ticket because it is just a scribble) gave me a ticket for speeding and failure to stop at a stop sign. I have always been a great driver, I never speed, I always stop at stop signs, I don't drink/text/talk over the phone and drive, I always have my seatbelt on. This one time I absolutely needed to get home at 2am I decide to go a little faster and yup, of course I get a ticket. I don't want to lie, I was doing 36 on a 20 zone at the time, I admit it even though I honestly did not realize I was going at 36 I didn't' do it on purpose. I didn't think hey I'm just gonna go faster because no one is around , I really honestly did not notice I was going at 36. But I am not sure about the stop sign thing. Honestly I was not paying much attention because I drive through this stop sign everyday but I distinctly remember there was a car already at the stop sign coming from the left so I HAD to stop. There was absolutely no one else coming from any of the four sides, so I might have not stopped completely but I did slow down almost to a stop and then kept going. Then like I said I was going at 36 on a 20 zone. This was at 2 a clock in the morning when there was literally not a soul around. The police officer was practically hiding in a dark, empty parking lot and came out of no where. I really don't want to make excuses for myself. I know what I did was wrong and I'll admit it. But this is my first ticket, and I really don't want t have to pay so much more money on my insurance for a slip as small as this one. My boyfriend was with mi in the car if that means anything. I really don't know, I've never had a ticket before. I've been reading up on how to get out of a ticket, and a lot of people say it is good to talk to the officer, and I thought about doing it but the officer's name is not readable at all. He also completely misspelled my name. In Puerto Rico we use both my father's and mother's last names so I have two last names. Let's say my name is X, my father's last name is Y and my mother's is Z. In Puerto Rico it goes like X Y Z and since my license is from Puerto Rico that is how it is written in my license. But the police officer wrote X Z. So my name is wrong in the ticket. I don't know if that counts for anything, someone told me that if they misspell my name or the officer's it means the ticket is dropped. This seems unlikely but hey, like I said I really have no clue. Again, I don't want to make excuses for myself, but I just can't afford extra insurance costs. I just cant...

What are legal steps to resolving an auto insurance claim?

Allstate was supposed to compensate me for X amount of damage done to my car by the driver who had insurance upto 5 grand. Notice that I did not have insurance with AllState but that because the driver was at fault, and had coverage of upto $5000, AllState has agreed to pay me a portion of that 5 grand. However, the problem is the driver who was at fault is not agreeing to this and demands more in loss from AllState. Whereas, although, I myself deserve more in damage since the car was worth more than the amount I am being compenstated for, I have still decided to settle for less. Now because the driver is delaying the decision to accept the remaining amount out of 5 grand, this case/claim has not resolved SINCE 2010. They wouldn't send me check yet only because the driver needs to accept the offer also. My theory is that whether or not the driver finds the offer fair, is none of my problem. I shouldn't have to wait for 2 years only because the driver refuses their offer. I should get my share at least. Do I have a legal case against the company for taking too long in this regards ? What can I do to resolve this ?

I'm 20 years old, have a clean driving record, and qualify for most merit-based discounts (no wrecks, good grades, etc.). I'm currently driving a 1998 Pontiac Grand Prix GT with a 3.8L V6, and paying about $800 per year listed as the primary driver of this car. I cannot possibly stress enough that I know having a newer car that I am still making payments on will increase my insurance. My question is basically if the change in price will be drastically higher than if I got full coverage on my current car. Beware this Scam on Car Insurance.!!?

A friend of mine was telling me about how his son was Scammed by a fake online Insurance Company. They are advertising car Insurance 50% cheaper, which of course is attracting young drivers. He paid 620 by his visa card for full cover, and was sent His Insurance documents. However, he was stoppd by the Police late at night for a Routine check, a few Months later, as It had come up on the NPR that he was driving without Insurance cover. He showed the Police his Documents and he was told that this company does not exist, and he is the 20th driver they have stopped and found that they have fake documents. However, the police did not press charges in this instance, but told him they have been trying to track these scammers for a whilr, but all they know is they are operating from Nigeria, and Latvia, but keep moving around. They advised the Lad in future to use a well known Insurance company or visit a good insurere in person to get Insurance. So God knows how many other drivers are on the road with useless Insurance cover.

About how much per month would insurance for a sub shop/pizza place cost an owner?

Doing research in attempt to know how much it will take to get a small sub shop off the ground. A big question is insurance. In this day in age, I would hate to be sued by someone who slips outside and falls. I will be renting the property from a local land owner. The building is 1,000 square feet, small joint. What is a general range of which I might in liability insurance for my business? Located in ohio small town.

I have a friend who is 31 years old, and is unable to find health insurance due to the fact that she had a heart attack a few years ago. They want me to pay $580 to get my insurance license before I even start. The company sells insurance and other financial options to consumers. They werent clear on my pay scale either. Mine was renewed {mine renews automatically) in April, but due to a change in finances, we started to look for something lower recently. We found one that is around $40 cheaper, plus offers roadside assistance. Do I have to wait until April to change it? If it does cost, what is the average charge? My husband's job has an insurance plan that will take $200 out of his paycheck EVERY OTHER WEEK. He makes about $1200.. that is $400 a month which leaves us with $800 a month to survive. Okay so a friend of mine hit my brothers car in our driveway today. We live in Alabama ... my friend has Alfa insurance and my brother has country financial. Both have full coverage. When we called to make the report the officer that came said that since it was on private property my friends insurance will not pay that my brothers insurance will.. my brother has a 500$ deduct able. So basicly I have 2 questions. 1.) Just because its private property why will my friends insurance not pay for it? 2.) How much will be played out of pocket because the damage is about 1500 if not more. (Its a new car 2011, the door is pushed in to the point the door can't open and window can't go down)
